Your Personalised Schedule of Care:
Your schedule of care is a personalised program that has been suggested by your doctor after assessing what condition your spine is in and evaluating your health goals. Your schedule has been designed to give maximum results. Your schedule of care is a living document and will be continuously reassessed throughout your care program. At your report of findings on your second appointment your doctor will detail your individual schedule of care. The number of visits recommended will depend upon the condition and severity of your problems, the length of time you have had the problem, your expected response to treatment and your own individual health goals/needs. If at any stage, you decide that your health goals have changed from what you have originally stated please inform the doctor at the earliest opportunity and alterations to your schedule can be made accordingly.
How often will I be re-assessed?
Your doctor will be reassessing your response to care on each visit. Formal reassessments of your whole health status will be conducted at regular intervals throughout your own individual program.
What if I have Questions about my Care?
All patients are encouraged to ask questions about their care. Your practitioner will have done their best in explaining your diagnosis, treatment, and prognosis; however, we understand that a variety of questions may arise. Should you have questions or require further information about anything in the clinic please do not hesitate to speak to either staff members or your doctor.
Reschedule/Cancelling appointments:
Your doctor has designed a care program that is specific for you. To meet your health goals, it is important to adhere to your program. If an appointment must be changed, we require 24 hours’ notice. All rescheduled appointments should ideally be made up later the same day or within 24 hours. Please advise our front desk staff a.s.a.p. so any changes can be made accordingly.
Missed/Broken Appointments:
If you do not turn up for an appointment without giving 24 hours’ notice the company reserves the right to charge you for the missed appointment at full price. Broken appointments should be made up within 24 hours. Please note – other clients cannot benefit from late cancellations.
Children, Family and Friend referrals:
Once you understand the important relationship between the spine, nervous system and general health, we expect that you will want to have your family checked. Any friends or family you refer to us will give you £10 free credit at Imperial Health (although not in conjunction with any pre- existing offers). If you know anyone who you think would benefit from our services, please let us know.
Refund Policy on Prepaid Plans:
If for any reason you wish to discontinue your care program a refund will be made. To calculate the amount of money you have used on your plan simply count the number of visits you have already had and multiply this number by £55 (our regular pay as you go adjustment fee), plus any products contained in your plan. This amount will be deducted from the money you have prepaid. Please allow 14 days for your refund to be processed. All applications for a refund must be made in writing to the practice coordinator. Please note that an administrative fee of £12.50 will be deducted from your refund amount. All pre-paid plans will be valid for a period of 18 months from the date of purchase.
Cash:
If you wish to pay for your care program by cash, the instalment needs to be fully paid by 00:00 hours of the payment day set up in your account, otherwise, the amount will be automatically charged to the credit/debit card provided and the sessions interrupted until the payment has been completed.
Access to Records Policy:
Under the terms of the data protection act (1988) we are obliged to tell you that the company will be creating a file on every client visiting our clinic. You have the right to access your file. However, by law we are required to retain your original records. Copies can be made as required upon request. Only the practitioners and the staff of Imperial Health Clinic will have access to your files.
Doctors reports:
We are happy to write you a report for yourself or for another healthcare practitioner but there may be a £50 charge depending on the time and effort put into it.
ZERO TOLERANCE POLICY
At the Imperial Health Clinic, we have the utmost respect for our staff, and we take it very seriously if a member is treated in an abusive or violent way, either in person or over the phone.
Thank you for understanding that aggressive behaviour and persistent, argumentative, or unrealistic demands that cause stress to staff will not be accepted, under any circumstances, and may result in you being dismissed from our practice.
